Hope Solutions is hiring a full-time Senior Case Manager for its Families in Supportive Housing Program to provide home-based case management and supportive mental health services to formerly chronically homeless families with special needs across Contra Costa County. The role performs psychosocial, life-skills, and financial assessments; develops and monitors service plans; coordinates medical, mental health, addiction, financial, legal, and other community resources; supports housing stability; and maintains HMIS/Apricot records. It requires a BA in social work or a related field, at least two years of human-service experience, an operational vehicle, valid California driver’s license, and the ability to work in the field and office. Compensation is $30 to $35 hourly, with Kaiser health and dental coverage, a 403(b) match, paid holidays, vacation, and life insurance.
