Description
The Senior Social Worker will provide psychosocial assessment, discharge planning, crisis intervention, counseling, psychoeducation, resource linkage, case management, advocacy, and documentation for patients and families in palliative medicine and geriatrics. The role manages a clinically complex caseload, supervises staff and MSW interns, participates in interdisciplinary rounds and projects, and may contribute to program planning. It requires a master's degree in social work and five years of post-master's experience in social services or healthcare, including at least one year of supervisory experience. The position is full-time, day-shift, based in Union Square, Downtown, and pays $53.97-$56.90 per hour.
