Description
CommonBond Communities is hiring a Service Coordinator - Tier 2 at Red Rock Manor to provide independent, person-centered service coordination for residents, including permanent supportive housing households when assigned. The role handles strengths-based assessments, service plans, crisis response, warm handoffs, community engagement, tenancy support, compliance, budget tracking, resident records, and collaboration with property management and community partners. It requires at least two years of experience working with diverse communities, knowledge of community resources, communication and teamwork skills, Microsoft 365 experience, and CPR and First Aid certification or certification within six months.
