Description
The Service Coordinator provides assessment, service planning, referral, coordination, and re-evaluation services for individuals with serious mental health issues, including persistent outreach, family and treatment-team collaboration, crisis planning, resource linkage, documentation, and advocacy. The role requires a bachelor’s degree in a listed field plus a Service Coordination Certificate, one to two years of human-service experience, relevant clearances, reliable transportation, and extensive travel. It is primarily an in-home, community-based position requiring a minimum 40-hour work week, nontraditional hours, and 24/7 on-call availability.
