Description
The Service Coordinator provides comprehensive case management and service coordination for adults with intellectual and developmental disabilities and their families. The role develops and implements individual Program Plans, coordinates medical, behavioral health, educational, vocational, recreational, and adaptive-device services, documents progress, initiates purchase-of-service requests, and collaborates with families, vendors, and community agencies. It is a community-based position requiring home visits, community meetings, and office work, with a 60% office and 40% community schedule.
