Description
The Service Coordinator provides comprehensive case management and service coordination for individuals ages 14 to 23 and their families, working with medical, behavioral health, educational, vocational, recreational, and other community resources. The role develops and implements individual Program Plans, documents service plans and progress, initiates purchase-of-service requests, and presents cases at specialized committees. It is a community-based position requiring 60% office work and 40% community or home visits, with a valid driver's license, reliable transportation, and minimum liability insurance required. The position pays $24.25 to $39.14 per hour.
