Description
The Service Coordinator provides comprehensive case management and service coordination for individuals ages 13 to 23 with developmental disabilities and their families. The role develops and implements individual Program Plans, coordinates medical, behavioral health, educational, vocational, recreational, and adaptive-device services, documents progress, initiates purchase-of-service requests, and presents cases to committees. It is a community-based position requiring 60% office work and 40% community or home visits, with a salary of $24.25 to $39.14 per hour.
