Easterseals Chicago Family Connections is hiring an entry-level Service Coordinator for its Early Intervention program. The role coordinates families from referral through case closure, manages case records and eligibility determinations, supports Individualized Family Service Plans, monitors service implementation, and assists families with insurance and other resources. The position is based at Child Family Connection #12 in Tinley Park, Illinois, requires a bachelor’s degree in a human or social service field, DHS credentialing, a valid driver’s license, reliable transportation, and home-visit capability, and pays $22.90 per hour.
