Easterseals Chicago Family Connections is hiring an entry-level Service Coordinator for its Early Intervention program. The role coordinates families from referral through case closure, manages case records and eligibility determinations, supports Individualized Family Service Plans, monitors service implementation and funding, and maintains family communication. The position is based at Child Family Connection #8 in Evergreen Park, Illinois, requires a bachelor’s degree in a human or social service field, DHS credentialing, a valid driver’s license, reliable transportation, and the ability to make home visits and perform physical tasks. The salary is $22.90 per hour.
