Pathways to Housing DC is hiring a Service Coordinator to support formerly homeless individuals with mental illness and co-occurring substance use disorders through Assertive Community Treatment. The role collaborates with a multidisciplinary team, develops Individual Recovery Plans, provides outreach, case management, counseling, advocacy, and tailored services across housing, health, employment, education, and recovery areas, maintains records, coordinates referrals, and leads groups and workshops. The position requires a bachelor's degree in social work or a related field or two years of relevant experience, strong communication and computer skills, a driver's license and vehicle, and COVID-19 vaccination; bilingual Spanish is preferred. The salary is $56,000-$60,000, with medical, dental, vision, retirement, and other benefits.
