Description
Rising Ground is hiring a full-time Service Referral Navigator to support the Clinical Support Program in Brooklyn. The role creates and maintains provider databases, identifies appropriate community-based services, makes referrals and appointments, provides warm handoffs, and coordinates with CPS and clinical teams. The position requires daily in-person work at Brooklyn ACS field offices, community travel, and an associate degree or higher; familiarity with Connections is preferred.
