Description
The Services Coordinator provides targeted case management and support to individuals with intellectual or developmental disabilities, including needs assessments, Individual Support Plan or Annual Plan development, service referrals, Medicaid and service-hour monitoring, support-system development, transition planning, and coordination with families, providers, and community agencies. The role requires knowledge of Oregon’s developmental disabilities public service system, a bachelor’s degree in a behavioral or social science field or an alternative education/experience combination, an Oregon driver’s license, and completion of required core competency training. It pays $26.72–$28.92 and requires a 40-hour workweek.
