Description
The Social Service Worker serves as an advocate and care manager for residents and families, conducting psychosocial assessments, care conferences, treatment planning, and communication with healthcare teams. The role provides emotional and social support, educates residents and families, coordinates ancillary services, advocates for resident rights, supports discharge planning, participates in quality improvement and staff training, and maintains documentation compliance. A bachelor’s degree in social work or a related field is required; 1–2 years of healthcare experience, especially in long-term care, is preferred, and LSW/LISW is desired.
