Description
The Social Services Designee supports residents and families during admission and discharge by managing referrals, screenings, tours, approvals, documentation, legal documents, emotional and psychosocial needs, advanced directives, and financial aid applications. The role also markets facility services, builds referral relationships, conducts social assessments and mental health screenings, develops individualized care plans, maintains records, ensures regulatory compliance, and contributes to quality improvement and policy development. The position requires healthcare, skilled nursing facility, or assisted living experience, strong communication and leadership skills, and the ability to pass a Minnesota Department of Human Services criminal background study.
