Description
The Social Services Designee supports residents, families, facility staff, and community agencies during admission and discharge by coordinating referrals, screenings, paperwork, discharge planning, home health, and equipment needs. The role provides psychosocial support, clarifies legal documents, participates in interdisciplinary care planning, completes social assessments and care-plan documentation, and maintains relationships with residents, families, staff, and agencies. It requires healthcare, skilled nursing facility, or assisted living experience and strong communication, organizational, management, and leadership skills.
