Description
Oak Park Care Center seeks a compassionate and thoughtful Social Services Director to integrate and coordinate social service/discharge planning activities within the hospital system. The director will develop and implement psychosocial assessments, formulate teaching plans, and demonstrate knowledge of clinical social work and discharge planning principles. This role involves interacting professionally with residents and families, providing individual and group support during crises, and coordinating the Social Service/Discharge Planning Department.
