Description
The Social Services Program Coordinator is responsible for overseeing all operational activities of the Social Services program, including planning, organizing, directing, coordinating, and managing services to meet member needs. The role involves case management to ensure clinically and financially responsible care, maintaining liaisons with other departments and community resources, and serving as a consultant to Tribal Option staff on social work matters. The incumbent will provide a wide range of social work activities to eligible Tribal Option members, facilitate positive professional relationships, assist with discharge planning, arrange member transfers, and make referrals for various resources, including financial, death and dying, advanced directives, and abuse/neglect. The position also includes developing care plans, ensuring conformance to social service and case management standards, implementing program activities, attending multidisciplinary rounds, coordinating member care, identifying necessary providers, interpreting member needs to medical staff, maintaining files, preparing reports, and participating in performance and quality improvement activities.
