Novocardia is hiring a full-time remote Social Work Coordinator based in Orlando, Florida. The role supports a multidisciplinary remote care team by assessing patients’ psychosocial needs and social determinants of health, developing care plans, coordinating referrals to behavioral health, financial assistance, transportation, housing, food, and other community resources, facilitating advance care planning, and maintaining EHR documentation and quality-compliant workflows. The position requires a Bachelor of Social Work, an active social work license, and 2–5 years of relevant healthcare, care management, care coordination, or community-based social work experience.
