Description
The Social Work Supervisor oversees care management services for children with chronic medical and behavioral health conditions, supervises staff, manages high-risk cases, coordinates referrals and care plans, monitors compliance and quality assurance, and conducts community outreach. The role requires behavioral health licensure, at least three years of related experience, interdisciplinary and care-management experience, computer proficiency, and availability for evening coverage and on-call duties. It is primarily office-based with substantial travel to members’ homes and community settings, and includes medical, dental, and vision benefits.
