Description
The Social Worker provides social services in a residential care facility, including resident and family counseling, social-needs assessments, care-plan documentation, family meetings, interdisciplinary coordination, resident-rights advocacy, financial-status assistance, and discharge planning. The role requires a bachelor's degree in social work or a related human-services field, compliance with New Jersey regulations, strong communication and interpersonal skills, and knowledge of governmental social-services requirements.
