CareOne at Madison Avenue is hiring a full-time Social Worker in Morristown, New Jersey, with a salary range of $25-$40 per hour. The role provides psychosocial assessment and therapeutic intervention, maintains medical records, develops care plans, coordinates interdisciplinary care, supports discharge planning, manages referrals and insurance verification, and serves as a liaison among residents, families, outside agencies, and the Center Administrator.
