Social Worker at Moravian Manor Communities

Moravian Manor Communities is hiring a full-time Social Worker to support residents of Saxony Ridge Apartment Building and Moravian Manor Communities Residential Living. The role includes completing social service paperwork, needs assessments, discharge planning, medical assistance coordination, care-transition support, and planning socialization and recreation activities. Applicants must have a bachelor's degree in social work or a related field, knowledge of the aging population, 3–5 years of experience working with seniors, and basic Microsoft Windows and Office skills.
