Description
Casa de las Campanas is hiring a Residential Social Worker to plan and coordinate psychosocial and case-management services for residents in independent, assisted living, and memory care units. The role supports residents and families through transitions, grief, caregiving, hospice, and aging-related issues; coordinates education and support programs; assesses cognitive, functional, and psychosocial abilities; and serves as a liaison for skilled nursing. The position requires a bachelor’s degree in social work, psychology, gerontology, or a related field, strong communication skills, and at least two years of experience working with geriatric residents in a long-term care setting. The schedule is Monday, Wednesday, and Friday from 8:30 a.m. to 5:00 p.m., with pay of $28.50 to $32.00.
