Description
PCHC is hiring a full-time, salaried Social Worker/Care Manager to support pediatric patients, caregivers, and providers through care coordination, assessments, referrals, advocacy, and connections to community resources. The role works with interdisciplinary teams, may include in-person, phone, and telehealth interactions, and requires a related social work or social services degree, at least three years of direct social work experience in a healthcare setting, community-agency experience, a valid driver’s license, and current or obtainable basic life support certification. The position is scheduled Monday through Friday from 8:00 AM to 4:30 PM, with possible schedule flexibility.
