Meriden Health & Rehab is seeking a Part-Time Licensed Social Worker to join their team. The successful candidate will be responsible for planning, organizing, developing, and implementing social services for residents, adhering to federal, state, and local guidelines. This includes completing admissions assessments, collaborating with an interdisciplinary team, providing support to residents and families, resolving grievances, coordinating discharges, and participating in quality assurance and safety meetings.
