Description
The Social Worker will assist the Social Services Director in planning, organizing, developing, and directing the overall operation of the Center’s Social Services Department. This includes assisting residents and families during transitions, conducting psychosocial assessments, identifying and meeting residents’ medical-related social needs, compiling resident social information, maintaining resource lists, implementing and evaluating social service programs, coordinating discharges, advocating for resident well-being, and ensuring accurate completion of assessment forms.
