Absolut Care of Aurora Park is hiring a Social Worker for its Long-Term Care team in East Aurora, New York. The role provides psychosocial assessments and ongoing support to residents and families, assists with care planning, discharge needs, care conferences, resident rights, and regulatory documentation, and coordinates with nursing, admissions, and community resources. A degree in Social Work is required, while Long-Term Care or Skilled Nursing experience is strongly preferred; knowledge of New York State Department of Health and federal long-term care regulations and strong communication, documentation, and advocacy skills are also required. The position pays $19.00–$22.00 per hour depending on experience.
