Description
The Social Worker supports the Columbus Correct Care initiative by coordinating care, advocating for vulnerable community members, navigating housing, food, and mental-health resources, conducting in-home and field assessments, and partnering with EMS, healthcare providers, and community agencies. The role also involves program development, data and HIPAA compliance, training, and flexible evening and weekend work. It requires a bachelor’s degree in a relevant field, at least three years of social-service experience, a valid Georgia driver’s license, U.S. citizenship or resident alien status, drug-screen and background-check clearance, and licensure as a Social Worker.
