Description
Sholom is hiring a Licensed Social Worker to provide resident services in its long-term care facility, including resident and family advocacy, ongoing assessment and evaluation, service coordination, and care planning. The role focuses on improving residents’ psychosocial, emotional, and behavioral well-being and reports to the Campus Director of Social Services. A bachelor’s degree in social work, gerontology, or a related field and unrestricted Minnesota social work licensure are required; one or more years of social work experience is preferred, preferably in a long-term care setting.
