Description
The Social Worker plans, organizes, and implements social work services for INSPIRA LIFE participants and families, including assessment, treatment, education, counseling, resource provision, mental health support, ongoing case management, advocacy, and disenrollment procedures. The role serves as a liaison among the interdisciplinary team, caregiver representatives, and community agencies, completes NJ Choice Level of Care assessments, and may work at the Vineland or Williamstown LIFE Program in Cumberland, Gloucester, or Salem Counties. A master’s degree in social work, New Jersey social work licensure, and at least one year of experience with frail or elderly participants are required; hospital, nursing home, PACE, and NASW certification experience are preferred.
