Touchstone Health is seeking a PRN Hospice Social Worker in San Antonio to provide psychosocial care to hospice and home-health patients and families. The role includes supporting patients through emotional, social, and environmental challenges; connecting them with community resources and financial assistance; collaborating on care plans; assisting with discharge planning; and documenting assessments and care coordination. Candidates need a CSWE-accredited master’s degree in social work, Texas licensure, at least one year of healthcare social work experience, and strong communication, assessment, advocacy, empathy, and independent field-work skills.
