This role involves contributing to care pathway development, educating patients and families, and acting as a key contact for continuous, coordinated care across various settings.

Ability to work independently and as part of a multidisciplinary team.
Non-medical prescribing qualification (if applicable to role).
Teaching or mentorship qualification.
