The SpEd Transition Coach independently plans and executes programs in a community setting to promote student independence, successful employment, and quality of life for students transitioning from high school to adulthood. This involves developing partnerships with various stakeholders, implementing IEP goals, assessing vocational interests, assisting with job placements, providing on-the-job training, teaching life skills, transporting students, monitoring program effectiveness, supporting peer relationships, solving behavioral and medical issues, informing students and parents about adult agencies, investigating new community resources, communicating student progress, educating community members on disability-related topics, attending IEP meetings, participating in special projects like Agency Fairs, serving on the Transition Steering Committee, attending monthly Transition Team meetings, maintaining detailed records, assisting in evening classes, and participating in ongoing training.
