Description
Swan Hospice is looking for a full/part-time Spiritual Counselor to provide non-denominational spiritual care to hospice patients, families, and caregivers. Responsibilities include assessing spiritual needs, consulting with patients and families, coordinating with community spiritual advisors, developing spiritual care plans, participating in interdisciplinary team meetings, maintaining records, and educating the IDT on spiritual care. The role requires strong communication skills, empathy, and adherence to agency policies and procedures.
