Description

The STAR Kids Transition Coordinator leads transition planning and service coordination for children with disabilities aging out of the Texas STAR Kids program. The role assembles transition teams, develops continuity-of-care and permanency plans, coordinates transitions to STAR+PLUS and other adult or community-based programs, supports relocation from nursing or intermediate care facilities to community settings, identifies providers and resources, completes assessments and documentation, negotiates for services and equipment, and ensures compliance with Medicaid, managed care, contract, and regulatory requirements. A bachelor's degree or equivalent is preferred, along with at least three years of transition or community-resource experience, or one year in managed care or case management.
