Description
The Stroke Program Coordinator is an experienced registered nurse who oversees care for stroke patients and supports seamless coordination with staff and physicians. The role includes data collection and analysis, continuous quality improvement, patient satisfaction initiatives, community outreach, patient education, guidance, and follow-up, while complying with The Joint Commission and other regulatory and accreditation standards. Required qualifications include at least five years of RN experience, a BSN, a current Illinois RN license, NIHSS certification, previous stroke experience, and proficiency with Microsoft Word, Excel, and PowerPoint; familiarity with TJC DSC for stroke and a CNS or APN are preferred.
