Description
The Stroke Nurse Navigator is a full-time Registered Nurse who provides individualized guidance, education, coordination, and support to stroke patients, families, and caregivers across the continuum of care. The role coordinates care from acute hospitalization through rehabilitation, outpatient follow-up, and community reintegration; delivers inpatient stroke education; supports stroke prevention and medication adherence; leads community outreach; and contributes to stroke program initiatives, quality improvement, regulatory compliance, and Comprehensive Stroke Center certification. The position requires a California RN license, BLS certification, recent relevant experience, and Microsoft Office proficiency, with CNRN or SCRN certification preferred.
