Identity is hiring a full-time SUD Senior Community Outreach Specialist/Case Manager to serve youth and families in Montgomery County, Maryland. The role manages an active caseload, conducts outreach and recruitment, connects clients with appropriate treatment, supports prevention and harm-reduction efforts, delivers workshops, documents outreach in the Efforts-to-Outcomes database, and responds to critical referrals. The position requires a high school diploma or equivalency, at least three years of relevant experience, knowledge of substance-use and recovery strategies, and proficiency with Microsoft Office, Adobe, Canva, and Zoom. It is a hybrid role with 60% remote and 40% in-office or on-site work, requires travel between sites, and pays $26.44 per hour.
