Description
The Supervisor, Care Coordination will oversee case management and related supportive services across the Youth Center, Interim Housing, and Permanent Supportive Housing programs. The role supervises case management staff, develops individualized development plans, coordinates referrals and housing resources, manages program operations, prepares reports, and collaborates with county agencies and community organizations. It also provides direct supervision to LGBTQ+ youth and requires substantial social-services experience, supervisory experience, a bachelor’s degree or equivalent training, and availability for evenings, weekends, and crisis situations.
