Description
The Support Coordinator works in De Singelhof, a modern apartment complex in Woerden serving 36 residents with intellectual disabilities. The role supports residents’ independence, maintains contact with their social networks, writes and implements individual support plans, contributes to team development, and helps create a safe and pleasant home environment. It is a permanent contract with a two-month probationary period, 24–32 hours per week including irregular and sleep-in shifts, and a gross monthly salary of €2,649–€3,980 effective 01-11-2026. Applicants need a Level 4 care diploma or relevant higher professional education, a Certificate of Conduct (VOG), and the ability to work as part of a team.
