Description
Hanover County is hiring a Support Coordinator to provide support coordination services for children and adults with intellectual and/or developmental disabilities, including co-occurring mental health and/or substance use disorder services. The role assesses needs, develops and monitors individual service plans, coordinates referrals and providers, supports community integration, maintains case records, and works independently and collaboratively. It is part of the Career Development Program, reports to a Support Coordination Supervisor or Program Coordinator, and does not supervise staff.
