Oswego County Opportunities, Inc. is hiring a full-time Supportive Housing Caseworker to provide service coordination and support to adults with severe mental illness in an NYS Office of Mental Health-funded program. The role manages a caseload of 30–35 consumers, conducts assessments and housing plans, monitors outcomes, provides on-site visits and community referrals, assists with funding and eviction prevention, maintains records and database entries, and coordinates with landlords, service providers, and agency staff. The position requires an associate’s degree in behavioral sciences or human services, one to three years of related experience, a valid New York State driver’s license, reliable transportation, and the ability to travel countywide.
