Description
The Target Case Manager assists families in planning and coordinating services for individuals with intellectual and developmental disabilities. The role attends IEP meetings as an advocate, provides community resources, develops person-centered support plans, conducts ongoing needs assessments, and coordinates with clients, community professionals, and coworkers. It is a full-time, Monday-through-Friday position in a professional office environment, with occasional travel between RUI sites, community agencies, and throughout the state.
