Description
The Time Limited Subsidy Case Manager provides housing-based support to homeless transition-aged youth aged 18–24, assessing needs, connecting them to appropriate housing, developing Individual Action Plans, coordinating health, education, employment, benefits, and other services, and maintaining case records and reports. The role carries a maximum caseload of 20–25 youth, requires regular face-to-face contact and home visits, and reports to the office five days per week. Candidates need relevant experience and a bachelor’s or associate’s degree, with a salary range of $50,000–$55,000 annually and medical, dental, and vision benefits.
