Description
Silverchain is hiring part-time TACP Home Care Workers to support older people transitioning from hospital back to independent living in the Lanceland and surrounds. The role provides personal care, exercise support, compression stocking assistance, transport to appointments, social support, meal preparation, and domestic assistance. It requires a Certificate III or above in Aged Care or Individual Support, at least six months of similar experience, a current Australian driver's licence, a reliable insured vehicle, and completion of mandatory vaccinations. The roster is Monday to Friday between 7 am and 5 pm, with availability for two to three days per week and minimal weekend work.
