Description
The MFP Transition Specialist provides information and outreach support for consumers on the MFP program, facilitates transitions from skilled nursing facilities, completes intake and transition planning, follows up for 365 days, submits Medicaid and other assistance applications, advocates for consumers, coordinates referrals, and maintains accurate records. The role requires a high school diploma or equivalent, at least two years of human-service experience, community-resource knowledge, Microsoft Office proficiency, reliable transportation, a valid driver’s license, and current automobile insurance. The stated salary is $18.50.
