Description
The Transition Coordinator plans, coordinates, delivers, and evaluates transition education and services for students with disabilities as they move from high school to post-secondary education, employment, independent living, or long-term support. The role conducts transition assessments and IEP-related planning, provides job preparation and career counseling, secures work placements, coordinates vocational training and community events, tracks graduates, and reports outcomes. It requires a bachelor’s degree in education, active certification in Special Education, Cooperative Education, or related education certification or School Counseling, and three years of experience working with a special needs population in a school setting.
