Description
The Lead Transition Specialist provides case management, individual service planning, crisis intervention, referrals, and follow-up to youth aged 16–21 receiving PAL Transitional and Financial Services. The role also mentors direct-reporting staff, maintains client files and Salesforce records, submits reports, and ensures compliance with DFPS, BCFS, and grant requirements. Applicants need a bachelor’s degree in social services or a related field, two years of relevant experience, First Aid and CPR certification, a valid Texas driver’s license, reliable transportation, and work authorization in the United States; visa sponsorship is not available.
