Description
Lancashire County Council is hiring a permanent, full-time Transitions Manager to lead its Young Adults Service team within Adult Social Care. The role manages and develops social care staff, oversees safeguarding, assessments, support planning, reviews, workforce performance, and strengths-based person-centred practice for young adults aged 18–25 with varied support needs. It also requires partnership working across education, health, employment, community, family, and care agencies, plus substantial post-qualifying social work experience and knowledge of adult social care legislation and transition processes.
