Upstate Caring Partners is hiring a Case Manager I for its Treatment Apartment Program (TAP), an OMH-licensed scattered-site housing initiative serving individuals with serious and persistent mental illness, co-occurring disorders, and housing instability. The role provides person-centered case management, treatment planning, housing and behavioral health support, crisis assistance, documentation, and coordination with treatment teams, families, landlords, and community providers. It pays $21.00–$25.00 per hour, typically follows a Monday–Friday 8:00 AM–4:30 PM schedule with possible evening or weekend work, and includes health, dental, vision, retirement, life insurance, paid time off, and other benefits.
